Job Description

MAIN RESPONSIBILITIES:

• To work within the Health and Wellbeing team to lead, develop and deliver new and existing programmes to support healthcare inequalities, in line with the Chelsea Foundation strategy and requirements set out by the NHS Trusts and funding providers. • To work closely with NHS partners and clinicians to identify local needs and develop appropriate support for those with long term health conditions within the local community.

• To act as lead contact for NHS trust project managers, co-ordinate and chair monthly operation meetings to ensure deliverables are being met according to the partnership requirements.

• To work collaboratively with the Health and Wellbeing officers to develop pathway activities to support the Foundation Wellbeing strategy in line with partnership insights and community need.

• To act as line manager to Exercise specialist and prepare delivery resources and session plans that support the project outcomes. Ensuring participation forms, recording sheets and registers are completed accurately and entered in CFCF's monitoring and evaluation systems. 


MEASURES OF PERFORMANCE:

• To be a positive role model, creating a safe, positive environment in which to motivate and encourage community members to lead a healthy active lifestyle.

• To build relationships where possible and relevant with local clubs, authorities, key community partners, youth services and health charities including local and national NHS trusts and services.

• To lead on the monitoring and evaluation for all Health & Wellbeing projects in accordance with the needs of each funder, including liaison with the Premier League Charitable Fund as a 'salesforce' user.

• Be responsible for the safeguarding of all participants taking part in Bridging the Blues programmes, liaising with the Safeguarding Lead for all associated partner and Safeguarding Team's to ensure a clear line of reporting to CFC Safeguarding Hub.

• Ensure success stories are captured through working with Chelsea FC Foundation/Chelsea FC media staff. 


SKILLS/KNOWLEDGE & EXPERIENCE

Qualifications:

Essential

• Relevant professional and/or academic qualifications or a minimum of 2 years' experience in the industry sector.

• A minimum level 4 Fitness Qualification (or willing to work towards)

• FA Safeguarding qualification or equivalent.

• FA Emergency First Aid Certificate or equivalent.

• Physical Activity and/or Sports Coaching Qualification

• Ability to travel to activity & meetings across South-West London 

 

Experience:

Essential

• Previous experience in working within a community health setting.

• Ability to build a strong rapport with partners and stake holders.

• Knowledge of community engagement programmes specifically tailored to long term health conditions.

• Proven effective project management experience. 


Desirable

• Knowledge of the Chelsea FC Foundation.

• Knowledge of local demographic and social needs.

• Experience of coordinating Sessional Staff & Volunteers.

• Knowledge of PLCF and PLPFA funded projects.

• Knowledge of Community Club Organisations and Football Charity Programmes.
