Job Description

Purpose:

Established in 1986, City in the Community Foundation is the registered charity of Manchester City FC and uses the brand of the parent Club to deliver award-winning programmes centred around Health, Education and Inclusion. The projects engage and impact over 20,000 lives in Manchester annually.

The purpose of the Community Outreach Mentor is to work within a child and young people (CYP) mental health Service (MHS) as part of a multi-disciplinary team delivering, under supervision, high-quality; outcome –informed, focused and evidence-based interventions for children and young people experiencing, mild to moderate anxiety, low mood, and behavioural difficulties.

 

Accountabilities with Key Outcomes:

Facilitate 1:1 mentoring interventions for CYP’s referred through PCN networks and complete a full transition support and signposting end of intervention package.

Support Mentoring Lead, Mentoring Officer, Safeguarding Officer, and Outreach Delivery Staff to ensure CYP’s needs are being met when at sessions and that interventions are safe and effective.

Support Project Officer in reporting on set KPIs and outcomes associated with the project to develop, maintain, and grow a strong working relationship with key stakeholders including the NHS, NHS FT, Manchester CAMHS, Practitioner care networks (PCN), Thrive navigators, Complex Safeguarding Hub and Children’s services.

Utilising own profile and community network to create, maintain and develop relationships with internal stakeholders in CITC and the wider City Football Group (CFG), local authorities and governing bodies to develop CITC’s reputation and reach, promoting all CITC programmes across Manchester.

Regularly review individual performance within the team to assess, identify and provide training and CPD opportunities which continuously improves the standard of delivery on the programmes.

Ensuring that CITC’s Safeguarding policy and procedures are embedded to the highest standard across the team. Work to ensure that safeguarding practices and principles are adhered to, ensuring our legal and moral obligations towards the framework.

Ensuring that all CITC and CFG policies and procedures are adhered to across the team, escalating any breaches as appropriate.
